Registered Parishner, Immigration, etc.

In order to request a certified letter, you MUST be registered with the parish for at least 3 months before you can receive a certified letter. If you are registered, you will need to submit a letter request form. Once the form is submitted upon approval it will take about 3 - 5 business days until its ready to be picked up.

New

All paperwork for NEW certificates must be turned in at least 2 weeks prior to the day the sacrament will be completed. Ex. All paperwork for a baptism (Copy of the child's birth certificate, copy of all three of the god parents' sacraments, baptismal registration form, and confirmation that the parents and God parents have completed the pre-baptismal classes.) Must be turned into the Parish office at minimum 2 weeks prior to the desired date of the baptism. Other Sacraments such as First communion, Confirmation, or OCIA, will be turned into the student's teacher unless stated otherwise. Paperwork for Marriage Certificates will be turned into the Father at the first marriage appointment. Certificates are made every 2nd and 4th Friday of the month.

Corrections

For corrections on certificates the ORIGINAL certificate MUST be returned in order for corrections to be made. Certificate corrections are made every 1st and 3rd Friday of the month. Corrections include typos such as mis-spelled names, incorrect names, incorrect dates, etc. If you are needing a name changed to a different name written in the book you will need to bring in a birth certificate that proves that is the correct name. For example, if at the time your child's name is Jane Doe on the birth certificate and for some reason, they have a name change to Mary Doe, the new birth certificate will need to be brought in, in order to change the name on the certificate and in the book. ALL certificates are made with the names on the birth certificate so please ensure everything is correct on the birth certificate.

Copies

For a copy of a certificate, you must turn in the request form on Thursdays before noon. Copies of certificates cost 20$ per certificate. Certificate copies are not copies of the original you received on the Sacrament so sometimes it may look a little bit different than the original. A certificate "copy" is a whole new certificate replacing the lost original. If you purchase a certificate copy and find the original the original is still valid, however if you are requesting a copy for a sacrament, please inform the office because it is a different kind of certificate copy.

Notations

Certificates with notations are for sacraments like marriage. For a certificate with notations, you will need to contact the parish office.
